Relax const uses in BossBoundaryEntry so the compiler can generate proper
copy assignment operator (error C2582: 'operator =' function is unavailable in 'BossBoundaryEntry')
Also renamed its members to match our coding standards
(public members should be UpperCamelCase)

- Fix a bug with Gormok and Sindragosa (Fixes#16280).
- Migrate boundary logic to Maps/AreaBoundary instead of having it sit in InstanceScript (to possibly allow use for other purposes).
- Implement the first five boundary types in Maps/AreaBoundary.cpp.
- Add boundary checks to Creature's update logic
- Add boundary data for all Northrend raids
- Add boundary initialization structures and methods to InstanceScript
- Modify EnterEvadeMode signature. It now passes a value from the EvadeReason enum as parameter to allow special casing depending on evade reason
- Remove previous (weird) boundary code that had them linked to GO spawns
- Rename Map::CanEnter to Map::CannotEnter. Return value changed from boolean (true means player can enter) to enum Map::EnterState (CAN_ENTER=0 means player can enter, any other value is a reason for deny).
- Move hack-y player error messages from within Map::CanEnter to the function calling CanEnter as appropriate (primarily WorldSession::HandleAreaTriggerOpcode).
- Modify WorldSession::HandleAreaTriggerOpcode to properly revive the player upon touching the portal leading to the instance they died in even if they are currently unable to zone in. Fixes and closes#15758.
- Modify Player::LoadFromDB to properly spawn players in the instance they logged off in if possible. Fixes and closes#15561.
- Modify permanent save behavior to be blizzlike: Players can always enter an instance they are saved to (assuming there are no map constraints against it), but get a homebind timer if the instance is already in use.
Fix doors opening/closing during Dreadscale fight by sending a CloseDoor command instead of toggling the door state when Dreadscale resets its movement.
Note that bounding the door state to boss movement makes poor sense and should be changed. Other bosses in ToC have this logic and should be updated if they show the same bug.
Fix some Flame Leviathan issues:
- Fix boss evading all the time due to bad Doors check
- Opened "Lightning Door" to other bosses only after boss dies
- Eject players from the vehicles when boss dies, make them untargetable and despawn them after 5 minutes (adjust the time to the blizzlike time of choice)
Move all InstanceScript initialization code from Initialize() to the constructor.
InstanceScript::Initialize() is now obsolete, please don't use it anymore. The only reason it's still in the sources is for backward compatibility.

Two minor changes in addition :
-Replace a defined value equal to 2^31 - 1 by std::numeric_limits<int>::max().
-Remove useless null-check on pointer returned by new. New doesn't returns nullptr on failure, it throws std::bad_alloc.
Create ScriptedAIs that require a InstanceScript reference only if the InstanceScript exists, so if these Creatures are in an instance. ScriptedAIs that don't require a InstanceScript reference have not been modified.
This fixes many possible NULL dereference crashes happening when spawning a scripted Creature outside of an instance.
Fixed a GetOwner() and a ToPlayer() NULL dereference crashes too.
- Logging System is asyncronous to improve performance.
- Each msg and Logger has a Log Type and Log Level assigned. Each msg is assigned the Logger of same Log Type or "root" Logger is selected if there is no Logger configured for the given Log Type
- Loggers have a list of Appenders to send the msg to. The Msg in the Logger is not sent to Appenders if the msg LogLevel is lower than Logger LogLevel.
- There are three (at the moment) types of Appenders: Console, File or DB (this is WIP, not working ATM). Msg is not written to the resource if msg LogLevel is lower than Appender LogLevel.
- Appender and Console Log levels can be changed while server is active with command '.set loglevel (a/l) name level'
